Health Information and Requirements

Information needed for September


The health desk at Monsignor Farrell is committed to helping our incoming students have a healthy and productive high school experience. To aid your son in a smooth transition into Monsignor Farrell High School, please assist us with the following items listed below that must be completed prior to the start of school in September.

 

1. 103 Health Card ("White Card")

Please follow up with your son's current school to ensure that his 103 Health Card (known also as the “White Card” which is managed by the school nurse), is transferred to Monsignor Farrell High School to my attention. The 103 Card is a medical record that is started/initiated whenever a student enters school in NYC and is updated by the school nurse. This card follows the student to each school the student attends. Therefore, your son's current school should forward his 103 Health Card to me. The 103 Health Card is vital to your son's overall health record while attending Monsignor Farrell High School.

2. Medical Administration Forms, MAF/DMAF

If your son has a chronic illness such as diabetes, asthma, food allergy, seizures, or other ailments, you are required to complete all relevant medical administration forms, such as the MAF, DMAF, and their related forms. These forms are completed with the assistance of your son's doctor. You can obtain these forms from your son's current school nurse or you can contact my office for assistance. The forms can also be found on the NYC Department of Education website.
