Required for All Freshmen and Students Transferring In
All freshmen parents and parents of students who are transferring in and will begin school at Monsignor Farrell in September 2021 must complete the Child and Adolescent Health Examination Form prior to the start of school in September. The form can be downloaded at the bottom of this page.
