Required for All Freshmen and Students Transferring In
All freshmen parents and parents of students who are transferring in and will begin school at Monsignor Farrell in September 2022 must complete the Child and Adolescent Health Examination Form prior to the start of school in September. The form can be downloaded at the bottom of this page.
You may complete the PDF form online, save it, and upload it using the button below, or you may complete the form by hand and bring it to the main office.
The deadline to submit the form is this Thursday, September 22. Any student who has not yet submitted the completed form will not be permitted to attend school or participate in any extracurricular activities after September 22 until the form is submitted.
