Monsignor Farrell

Diploma Request Form

To receive a reprint of a diploma, Monsignor Farrell requires all alumni to submit two forms of identity verification (SSN and photo ID).
Diplomas are $40 per copy and can take up to two weeks to print.
Diplomas can be picked up in the Main Office at Farrell or mailed to you.

Once this form is completed and your identity is verified, you will receive a payment link via email.

After payment is received, you will receive an email notification when your diploma is ready to be picked up, or when it was mailed.
