All Students
All student-athletes must have a completed FinalForms account and approved medical documentation before they are allowed to participate in any Monsignor Farrell High School athletics/team activities, including workouts, tryouts, practices, and/or games.
Please complete the following steps:
- Access FinalForms: https://monsignorfarrell-ny.finalforms.com/
- Print the NYCHSAA Preparticipation Physical Evaluation Form.
- Have the physical completed and signed by a licensed physician.
- Upload the completed physical to FinalForms.
- Once the physical has been uploaded, the Monsignor Farrell Athletic Department will verify the documentation and update your son's physical expiration date.
