Inform the School Nurse
Any student with chronic illnesses such as food allergy, asthma, diabetes, seizures, and other chronic illnesses should complete the relevant Medical Administration Form (MAF) and accompanying forms.
- The MAF provides the school nurse with prescribed orders to treat your child during the school day.
- Even if your son can take his own medicine, it is important that the school nurse be aware of the student’s medical needs in case of an emergency.
- You must submit a new MAF each school year.
These medical forms can be obtained from the Department of Education (DOE) website, or students may pick up forms in the Monsignor Farrell High School Nurse's Office.
