New for 2023-2024
The Monsignor Farrell Department of Athletics is now using FinalForms, an online forms and data management service.
All students who are interested in participating in a sport, including intramurals, must register with FinalForms. No student will be permitted to attend workouts, tryouts, practice, or games unless he has a FinalForms account and has submitted all required forms.
FinalForms allows you to complete and sign athletic participation forms online and saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and update them after every season of participation.

We are requiring all athletic documents to be submitted on FinalForms.
